Description

POSITION SUMMARY:

The Case Manager will refer clients to individual, group, and family group counseling services provided by WCGC or other appropriate service provider and will complete documentation required for the clinical records. This includes but is not limited to development of the case management reentry plan, assessments, daily notes, and monthly reviews. The Case Manager will provide case management services including but not limited to exploring and identifying community resources, facility communication between community resources and other treatment providers, coordinate services and assist clients in obtaining benefits. The case manager will also deliver evidence-based curriculum. Essential Job Functions include those listed below.

ESSENTIAL JOB FUNCTIONS:

  • Heavy case management of a large caseload of participants.
  • Builds and maintains relationships with property owners/managers to secure units for leasing.
  • Meet with each client at least once weekly to provide support and assistance towards achieving each Individualized Service Plan
  • Intake and assessment of incoming participants to determine eligibility and appropriate services.
  • Make referrals and appointments with appropriate agency, services and/or case managers.
  • Maintain client files according to company policy and contract requirements.
  • Heavy documentation; HMIS data entry
  • Provide daily, weekly and monthly reports as requested.
  • Assist and monitor data collection and statistical reporting and complete Quality Improvement reports as requested.
  • Maintain relationships with referring and supporting agencies and represent the agency at various community meetings as requested.
  • Coordination of various services throughout several agencies.
  • Attend scheduled meetings and presentations with referral agencies; and
  • Perform any other duties as assigned.
  • Must Embrace and embody the mission, vision, guiding principles, clinical vision, and goals of WestCare Foundation; and
  • Perform all other duties as assigned.
Qualifications

ESSENTIAL QUALIFICATIONS:

To perform this job successfully, an individual must be able to perform the duties and responsibilities satisfactorily. The requirements listed below are representative of the knowledge, skill, and/or ability required.

Experience and Competencies:

  • Excellent interpersonal and communication skills.
  • Ability to work independently and as part of a collaborative team.
  • Compassion, empathy, and a non-judgmental approach towards individuals with substance use disorders.
  • Understanding to talk to and communicate with clients in an effective and appropriate manner.
  • Bilingual/bicultural (English/Spanish) is preferred.
  • Adherence to the highest standard of ethical conduct, especially to standards governing confidentiality.
  • Professional appearance and demeanor.
  • Must be culturally/linguistically sensitive to populations served.
  • Must be able to obtain and maintain level II background clearance.
  • Valid driver's license and ability to obtain and maintain WestCare's vehicle insurance is required.
  • During your tenure with WestCare there may be new requirements, including, but not limited to vaccinations that are issued by local, State, Federal, and/or Funders that WestCare may have to comply with. Should this occur Human Resources or appropriate personnel will inform you.
  • CPR Certification, First Aid Certification, and an annual Tuberculosis test.

EDUCATION and/or EXPERIENCE:

  • Two (2) years' experience working in substance abuse prevention, community health promotion, or related fields is highly desirable.
  • A strong understanding of substance abuse issues, mental health promotion, and evidence-based prevention strategies is essential. Familiarity with theories of behavior change, public health principles, and community organizing approaches is advantageous.
  • Bachelor's degree from an accredited university or college with a major in counseling, social work, psychology, nursing, rehabilitation, special education, health education, or related human serviced field is required.

LICENSURE/CERTIFICATION:

  • Not applicable for this position.

WORKING CONDITIONS:

  • Work is primarily performed in an office or in a Residential/Outpatient treatment setting.
  • Travel throughout and around the community and outreach is required.
  • This position may require flexible hours, including evenings and weekends, to accommodate client needs and community events.
