JOB DESCRIPTION

POSITION TITLE: Nurse Practitioner, Family Health Center of Marshfield, Inc.

JOB SUMMARY

The Nurse Practitioner (NP) is a skilled member of the health team who provides direct preventive and primary care for pediatric, adults and geriatric patients under collaboration with a licensed physician, according to Nurse Practitioner Protocol, and within the scope of the Nurse Practitioner License in the State of Wisconsin. Family Health Center provides comprehensive, integrated primary health care services for all, regardless of one’s background or circumstances. Care provided is related to health maintenance, acute care, and chronic disease management at the health center or in outreach settings including mobile services.

ESSENTIAL JOB FUNCTIONS
  1. Provides direct medical care, including:
    • Complete medical history
    • Physical examination
    • Recommended screenings
    • Diagnoses
    • Plan of care including appropriate treatment and/or referral
    • Follow up and ongoing care
  2. Provides high-quality, inclusive, equitable and culturally and linguistically appropriate care.
  1. Orders and uses all available resources in diagnosis and treatment: i.e. labs, x-ray, in an appropriate and cost efficient manner.
  2. Confers with other clinical team staff members, outside experts when necessary, and with patient family, as appropriate, concerning treatment and care of patients and completion of prescribed care.
  3. Refers patients in need of additional diagnostics, specialty care or support services to appropriate referral source, reviews referral results in a timely manner and communicates result to patient.
  1. Provides appropriate information to individual patients and families concerning medications, plan of care, treatment regimens, and health education using effective communication.
  2. Is a collaborative member of the care team that includes medical support staff, case management enabling services, telephonic pharmacy care, multi-disciplinary colleagues (A&DRC, Dental etc.)
  3. Identifies barriers to patient care flow or delivery and assists in formulating corrective action.
  1. Thoroughly and correctly documents medical encounters in electronic medical records system.
  2. Correctly codes medical encounters according to level of patient encounter.
  1. Adheres to drug formulary used by the practice to be compliant with agreement.
  2. Supervises and intervenes with clinical issues and supports/directs triage by clinical staff when necessary.
  3. Follows established safety precautions and procedures in the performance of all duties in order to ensure a safe environment.
  4. Regular attendance is required in order to carry out the essential functions of the position.
  1. Reviews and meets ongoing competency requirements of the role to maintain the skills, knowledge and abilities to perform, within scope, role specific functions.
ADDITIONAL DUTIES
  1. Prepares written prescription orders for drugs and controlled substances.
  1. Triages patient telephone calls and troubleshoots questions to ease concerns of patients and family members.
  1. Actively participates in selected committee activities.
  2. Other duties as assigned.
JOB QUALIFICATIONS EDUCATION

For positions requiring education beyond a high school diploma or equivalent, educational qualifications must be from an institution whose accreditation is recognized by the Council for Higher Education and Accreditation.

Minimum Required: Master’s Degree in Nursing.

EXPERIENCE

Preferred/Optional: Three to five years’ nursing practitioner experience.

CERTIFICATIONS/LICENSES

The following licensure(s), certification(s), registration(s), etc., are required for this position. Licenses with restrictions are subject to review to determine if restrictions are substantially related to the position.

Minimum Required: Current State of Wisconsin Nurse License or Nurse Compact License, and Nurse Practitioner Licensure. Current Drug Enforcement Administration (DEA) License. Mobile Care: Basic Life Support (BLS) certification awarded by the American Heart Association (AHA). Or clinic: Advanced Cardiac Life Support (ACLS) awarded by the American Health Association (AHA) at time of hire. DATA-waiver prescription privilege (DEAX). Additional department-specific certifications, licenses, or memberships may be required (varies by department/location).

PATIENT POPULATIONS SERVED

Individuals hired are expected to serve patients of different ages, backgrounds, etc. When performing the essential functions of the role, the individual must identify the patient age group to be served and apply the appropriate care based on the patient’s age and background. The hiring manager is responsible for communicating the patient population generally encountered in the role and is responsible for ensuring the appropriate cultural, age and related training needed to serve the patient populations.
