Join Rise Health as a Nurse Practitioner or Physician Assistant, where you'll collaborate within an integrated care framework to deliver top-tier primary care services. Your pivotal role will encompass:

Exceptional Patient Care:
-Administering personalized, expert care directly to patients
-Maintaining precise and up-to-date patient records Medical Proficiency:
-Analyzing current patient symptoms and determining diagnoses for care management and active medical treatment
-Conducting physical examinations, medication evaluations, wound care, and depression screenings

Professional Collaboration:
-Working harmoniously with fellow clinical providers, APPs, and physicians in a 100% outpatient primary care clinic
-Effectively communicating assessment findings to keep the PCP informed about potential care gaps

- Graduation and certification from an accredited advanced practice program are prerequisites
- A track record of two or more years in outpatient clinical settings as an Advanced Practice Provider, demonstrating autonomy in care provision without physician oversight
- Proficiency in treating adult patients, conducting comprehensive health histories, and performing physical assessments for patients and families with acute, chronic, or potential health issues
- Possession of a current, unencumbered State license or eligibility for licensure
- Active registration with the Drug Enforcement Agency (DEA) or eligibility for registration
- Current Medicare/Medicaid Enrollment or eligibility to enroll is required
- Certification in Basic Life Support (BLS) or Advanced Cardiac Life Support (ACLS)
- Effective verbal and written communication skills
- Comfortable navigating Electronic Medical Record (EMR) systems
- Basic proficiency with Microsoft Office tools
