Join our dynamic healthcare team! We are currently seeking a Clinical Pharmacist for a rewarding opportunity to enhance patient care. If you are passionate about optimizing medication therapy and thrive in diverse clinical settings, we want to hear from you!
Key Responsibilities:
- Participate in direct patient care.
- Manage medication therapies.
- Evaluate drug efficacy
- Order and evaluate laboratory tests
- Medication history and counseling
- Communicate directly with healthcare providers regarding medication management and therapy adjustments.
- Provide drug information to other healthcare professionals and responding to provider initiated.
- Responsible for satellite pharmacies, which may include tasks such as sterile product processing in the IV Admixture Service and OR pharmacy.
Shift:
- Monday-Friday: 7:30am-4:30pm
- Rotating weekends
Qualifications:
- B.S. in Pharmacy required; Pharm.D degree preferred.
- ASHP-accredited PGY-1 Pharmacy Practice Residency preferred; or 3 years of acute care hospital experience
- Board certification preferred.
- DE Pharmacist license
