JOB SUMMARY:
This position serves the hospital patient population by observation and monitors cardiac rhythms/capnography, recognize changes in regularity, configuration and rate of rhythm and report findings to primary nurse. Appropriately documents and communicates monitored recordings. This position also includes clerical functions and specific patient care duties to facilitate efficient and comprehensive services and patient care in critical care areas. The Nurse Director/House Supervisor may re-assign to other units based on changes of volumes and acuity.

JOB DUTIES:
Clerical:
1. Accurately identifies and enters the patient information: initials, telemetry box number into the monitor system and telemetry/capnography log.
2. Assembles/disassembles patient shadow charts in a timely fashion to facilitate patient flow.
3. Promptly and accurately responds to delegated tasks within scope of practice.
4. During EHR downtime, accurately and promptly completes necessary paperwork which assists to ensure tests, consults and treatments are initiated/completed.
5. Reports appropriate overhead pages via the paging system to the Admitting staff in accordance with established organizational procedures.
6. Completes after visit summary (AVS) and any additional discharge or transfer process.
7. Maintains a clean and organized workstation.
8. Identifies all patient information using HIPAA standards.
Patient Care Duties:
1. Assists in the delivery of patient care as directed to include: transfers, ambulation, feeding, dressing, bathing, weighing, and toileting.
2. Promptly delivers specimens and supplies within the hospital.
3. Responds promptly to all patient safety alarms, i.e. call lights, bed and chair alarms.
4. Applies monitors and reports to the RN the effectiveness of any safety device and/or mobility alarm.
5. Participates with the RN in rounding for patient safety checks and customer service.
6. Initiates the cleaning of patient care areas upon discharge and prepares the room for new admission by disinfecting, changing of linen, restocking room supplies, removing garbage and readying equipment.
7. Notifies primary nurse immediately of significant changes in monitor patterns and follows the chain of command if no response or inappropriate response occurs.
8. Accurately recognizes and reports life threatening arrhythmias (Ventricular Tachycardia, Ventricular Fibrillation, Asystole).
9. Accurately interprets and measures intervals of the cardiac rhythm and other parameters upon admission, at the beginning of each shift, and with significant changes.
10. Advocates for the patient by ensuring appropriate alarms are on and limits are set - individualized for the patient in collaboration with the RN or physician or defers to default settings when appropriate.
11. Alert to alarms when not directly in eye sight of monitor while initiation NA duties.
12. Advocates for patient safety by troubleshooting problems at the bedside; performing battery replacement, ensuring the replacement of electrodes to achieve the best possible waveform in a timely manner.
13. Demonstrates accurate interpretation of monitors including appropriate documentation and communication of monitored recordings to the primary nurse.
14. Provides the primary nurse with a printout of a rhythm strip (with a clear baseline) on admission, at the beginning of each shift, and with significant change in rhythm for documentation in the patient's medical record.
General:
1. Demonstrates knowledge of and compliance with hospital safety, disaster and emergency procedures.
2. Cleans and disinfects equipment as needed keeping unit clean and organized.
3. Attends required in-services and a minimum of 50% yearly attendance at unit staff meetings.
4. Understands and adheres to departmental policies and procedures.
5. Care is delivered with adherence to the policies and procedures of Grand Itasca Clinic and Hospital.
6. Performs and demonstrates competency of CNA responsibilities for patient care.
7. Excellent communication. Teamwork, collaboration, and interpersonal relationship skills required
8. Ability to sit observing monitoring for extended periods of time
9. Orders and maintains supplies needed for monitoring.
10. Demonstrates proper maintenance and inventory of all equipment to facilitate optimal working condition.
11. Inventories and maintains monitor equipment: portable Spacelab monitors and hardware monitor equipment for daily use.
12. Maintains equipment inventory log daily.
13. Reports missing equipment to the Biomedical for replacement or reports issues to charge nurse or supervisor.
14. Removes equipment from patient use when in need of repair, collaborates with Biomedical engineering and inventories for equipment return. Completes maintenance request order.

QUALIFICATIONS:
1. Must be 18 years of age or older.
2. Graduate of a Nursing Aide or EMT program. a. Current certification of the Minnesota Nursing Assistant Registry or certification through the National Registry of Emergency Medical Technicians (EMS's) or Minnesota Emergency Medical Services Regulatory Board is preferred.
3. One year of relevant medical education/experience including hands-on patient care on healthcare environment required.
4. Basic ECG training, reading and interpretation at time of hire or within 60 days of formal telemetry training preferred.
5. ECG certification or obtain within 60 days.
6. BLS certification or ability to obtain within two months of hire and maintain certification.
7. One-year telemetry monitoring experience preferred.
8. Experience in an ambulatory or inpatient health care setting with ability to multi-task and manage multiple patient/family/nursing/physician priorities within short time frames preferred.
9. Demonstrated ability to adhere to and follow Grand Itasca Clinic & Hospital Mission, Values, Vision and policies.
10. Maintains reliable and consistent attendance.
