JOB SUMMARY\n
- The RN Care Manager is a member of the interdisciplinary group (IDG) who coordinates the patient's care, 24/7 across all settings (home, hospital, nursing home, assisted living, and group homes), to ensure continuous assessment of the patient's and family's physical, psycho-social, spiritual, and end-of-life care needs and implementation of the Hospice's palliative plan of care. Ensures optimal team care coordination to facilitate comfort, meaning, purpose in patient/family end of life closure experiences. All activities will be performed in support of the mission of Phoebe Sumter Hospice.\n
- Adheres to the hospital and departmental attendance and punctuality guidelines.\n
- Performs all job responsibilities in alignment with the core values, mission and vision of the organization.\n
- Performs other duties as required and completes all job functions as per departmental policies and procedures.\n
- Maintains current knowledge in present areas of responsibility (i.e., self education, attends ongoing educational programs).\n
- Demonstrates competency at all levels in providing care to all patients based on age, sex, weight, and demonstrated needs. For non-clinical areas, has attended training and demonstrates usage of age- specific customer service skills.\n
- Wears protective clothing and equipment as appropriate.\n
- Must possess and maintain a valid Georgia Driver's License, reliable transportation, and current automobile liability insurance for travel to patient's home and must furnish report of Motor Vehicle Records for the past three (3) years.\n
- Must spend 50-75% of work time in patient's residence.\n
- Presents a professional appearance in accordance with established policy.\n
- Facilitates and Attends staff meetings and completes mandatory in-services and requirements and competency evaluations on time.\n
- Ensures all clinical care is conducted in compliance with all state and federal hospice regulations.\n
- Actively participates in the hospice's Quality Assessment and Performance Improvement (QAPI) program.\n
- May be exposed to high noise levels and bright lights.\n
- May be exposed to limited hazardous substances or body fluids, or infectious organisms.\n
- May be required to change from one task to another of different nature without loss of efficiency or composure.\n
- Periods of high stress and fluctuating workloads may occur.\n
- May be scheduled as needed including overtime.\n
- General environment: Works in a well-lighted, air-conditioned area, with moderate noise levels and in patient/family homes.\n
Qualifications
EDUCATION REQUIREMENTS\n
- 2 year / Associate Degree in Nursing from a state accredited school (Required)\n
- 4 year / Bachelor's Degree in from a state accredited School of Professional Nursing. (Preferred)\n
- 2 - 3 years General Medical Surgical Nursing experience. (Required)\n
- 1 - 2 years Hospice or Home Care Experience. (Preferred)\n
- Required Certifications/Licensures: Registered Nurse (RN) with current Georgia license, Certified Case Manager (CCM) or Certified Hospice and Palliative Nurse (within 3 years of hire)\n
