RN Case Manager positions available will be at these locations:

• Providence Little Company of Mary Torrance Hospital, Torrance, CA
• Providence Little Company of Mary San Pedro Hospital, San Pedro, CA

The RN Case Manager performs the primary functions of assessment, planning, facilitation and advocacy, which are achieved through collaboration with the client and other health care professionals involved in the client's care.

Required qualifications:

Care Manager RN

• Bachelor's Degree
• Upon hire: California Registered Nurse License
• 2 years Acute hospital experience, including one year of supervisory experience.

Care Manager MSW

• Master's Degree Social Work from an accredited program.
• 1 year Experience in healthcare related field (Acute, Ambulatory, Post-Acute, etc.) which includes the completion of an MSW intern program in a healthcare setting.
• 1 year Experience in Acute Care Management (Care Coordination) or successful completion of the Transitions in Practice (TIP) program for Care Managers

Care Manager LCSW

• Master's Degree in Social Work.
• Upon hire: California Clinical Social Worker License
• Within 1 year of hire: CPI certification (nonviolent crisis intervention training)
• 2 years of experience in healthcare related setting with psychiatric mental health patient population or within an emergency department working with psychiatric patients.
• Computer experience as needed for the job.
