Position Summary 

Improves overall quality and completeness of clinical documentation by performing daily medical record review.

Conducts timely follow-up reviews of clinical documentation to ensure that the issues discusses and queries left in the medical record have been answered by the provider.

Queries physician on specificity of procedure performed and diagnosis based on accepted coding guidelines and clinical expertise.

Conducts education sessions with physicians and other health care team members on documentation requirements.

Reviews clinical issues with physician advisors.

Works closely with case management, quality management, risk management and medical staff credentialing to provide data related to key clinical indicators and operational metrics.


Minimum Qualifications 

RN Qualifications

    • Minimum Education: Graduate of a School of Nursing program.  
    • Minimum Experience:  Prior experience in case management, clinical documentation improvement, and/or coding accuracy preferred. Minimum of five (5) years experience in a clinical role and minimum of two years experience with inpatient coding, process improvement in an acute care facility preferred or equivalent. 
    • Licensure/Certification: Current New Mexico or compact RN license required. Certified Documentation Improvement Specialist (CDIS) preferred.  

Coding skills with experience in ICD-9-CM, knowledge of CMS Inpatient Prospective payment System, and working knowledge of one or more of the following: Medical/Surgical Nursing, Critical Care, Case Management (Resource Utilization), Surgical Services, Perinatal Services Accreditation and Regulatory Compliance, Core Measure and Public Reporting of Hospital Quality Date. Experience in development of reference bases continuing educational programs using Adult Learning Principles.

Coder Qualifications

    • Minimum Education: High School diploma or equivalent. Formal education in ICD-10 coding required. Formal education in medical terminology, anatomy and physiology or equivalent experience.
    • Minimum Experience: Medical office or Health Information Management Department experience in coding. Working knowledge of ICD-CM and CPT coding standards and practices.  
    • Certification/Licensure:  AHIMA certification strongly preferred. AAPC certification recognized. Must become certified within one year of hire date if not certified at time of hire.
