DESCRIPTION OF WORK

This is a trainee position (probationary period of six months) within the Luzerne/Wyoming Counties Area Agency on Aging. This employee is specifically trained to assess the needs of consumers utilizing state mandated assessment forms, develop effective care plans to meet these needs, and implement these care plans in a professional manner.

This position will be responsible for completing Reports of Need on consumers aged 18 and older, after successful completion of required training courses. In this role, you will make priority home visits for safety and welfare check of consumers as required, and work as a liaison between the consumer and provider, as necessary.

The Aging Care Manager 1 will also assist consumers, guardians, and families in maximizing their abilities for self-determination by enabling and empowering them in decision-making to the greatest extent possible. Work involves reassessing consumers on an annual or bi-annual basis, depending upon the care program the consumer is enrolled in, identifying changes in situation or function, measuring progress toward the goals, and educating consumers and their families regarding aging services and benefits.

Work Schedule and Additional Information:

  • Full-time employment.
  • Work hours are 8:00 am to 4:00 pm, Monday - Friday, with 30-minute lunch.

REQUIRED EXPERIENCE, TRAINING & ELIGIBILITY

QUALIFICATIONS
Minimum Experience and Training Requirements:

  • Three years as an Aging Case Aide 2; or
  • A bachelor's degree which includes or is supplemented by twelve (12) credits in social sciences, behavioral sciences, human services, or a closely related field; or
  • An equivalent combination of experience and training.

  • Hiring preference for this vacancy will be given to candidates who live within Luzerne -Wyoming County. If no eligible candidates who live within Luzerne-Wyoming County apply for this position, candidates who reside in other counties may be considered.
  • You must be able to perform essential job functions.
