DESCRIPTION:

Position: Licensed Practical Nurse (LPN)

Full-Time/Part-Time: Full-Time

Shift: Days

Rate of Pay: $20.00 - $27.00 per hour

Exempt/Non-Exempt :Non-Exempt

If you are interested in direct patient care in an exciting Family Medicine setting, please consider CNY Family Care. Our LPN's have the pleasure of working with the patient, from start to finish, and implementing our preventive care measures so you are actively involved in the health and wellbeing of our patients.

We are seeking full time LPN's to join our practice and the position is to include days, some evenings and occasional weekends. 10% shift differential pay for evening (5pm - 9pm) weekday and weekend hours worked.

Responsibilities include: Obtaining vitals, intake process, working with provider to assist with patient visit, administering injections, performing ECGs, telephone triage, placing orders and refilling Prescriptions at the Providers request and Phlebotomy.

Job Requirements:

  • Current Valid New York State Licensure as a Licensed Practical Nurse.
  • Prior experience in busy medical office preferred, but not required
  • Strong critical thinking skills
  • Excellent customer service skills and the ability to work in a fast paced, team environment.

One of our main goals at CNY Family Care, LLP is to provide all of our patients with continuous and comprehensive health care. As part of this care model, we ask all of our patients to select one personal physician. This PCP/Primary Care Physician will maintain an ongoing relationship with the patient and will then always be in charge of their care. They will oversee the care team that consists of nurses, Nurse Practitioners, Physician Assistants and Diabetic Educators who might interact with the patients when they visit the office. Our care team will work towards ensuring that the patients receive the appropriate age-related preventative testing and treatment as well as the latest evidence-based treatment (proven and documented) for their individual medical condition(s).
