Summary:

Full-time member of Outreach team responsible for direct service, and facilitation of educational programming and case management at Lackawanna County Prison and 7th Avenue Center for the Independent Living Program. The case manager is responsible for day-to-day activities associated with Outreach’s young adult programming.

Duties:

? Plans, conducts, and delivers high quality program services within the scope of current funding goals.

? Assists in the development of individualized transition plans for youth 14 years of age and older accepted into the program and/or meeting eligibility requirements.

? Utilize current assessment tools in conjunction with family group decision making conferences, and family team meetings to create individualized family service and transition plans for youth.

? Provide instruction on employment opportunity development and placement, training, interview skills, resume preparation and application completion.

? Complete financial aid applications with youth in post-secondary educational settings.

? Facilitate agency independent living psycho-educational support groups for participants ages 14-23 utilizing approved curriculum; educate youth on consumer awareness, responsibility, and finances.

? Facilitate services for expectant parents and coordinate with school and community based programs education and preparation.

? Assist youth, both individually and as a group, with visiting prospective college campuses across the state.

? Plans and delivers life skills educational presentations to increase students’ knowledge; prepare outlines of instructional programs; studies and assembles material to be presented.

? Provides individual case management and necessary services to ensure all young adult needs are being met; advise and monitor participant progress; track program and individual goals; refer out for additional community support as necessary.

? Collaborate with other staff and team members by discussing assessment outcomes and reviewing individual client progress.

? Assists program manager in the evaluation and selection of program curriculum and instructional materials with emphasis on research/evidence-based strategies for individual training, group instruction, self- study, lectures, demonstrations, simulation exercises, role play, and computer-based training.

? Participate in various subcommittees geared towards assisting youth, as needed on the local, regional, or state level.

? Completes timely documentation, data collection, management and analysis; maintenance of case files; and outcomes reporting for the young adult program.

? Ability to maintain confidentiality; establish and maintain effective working relationships with other staff, officials and other ancillary professionals.

? Must operate under the guidelines of Outreach and Lackawanna County Prison.

? Evening and weekend work will be required.

? Participates in weekly team meetings and any agency team meetings.

? Performs other duties as assigned.

? Daily attendance is mandatory.

Qualifications:

Education/Experience:
Bachelor’s Degree, or 3-5 years related experience and/or training; or equivalent combination of education and experience. Helpful experience and/or background to include, experience in human services; familiarity with probation/parole, office of youth and family services & the court system; experience working with youth and at-risk populations. Knowledge of adolescent stages of development, life skills, vocational skills, consumer awareness, hygiene issues, sexuality, individual legal rights, housing, nutrition, parenting, and financial aid services helpful.


Computer Skills:
To perform this job successfully, an individual should have knowledge of Microsoft Office software and database software.

Certificates and Licenses:
* Must have, or meet the requirements to obtain, current ACT 33 (Child Abuse) & 34 (Criminal Background) Clearances.

* Must be able to obtain additional clearances as required.

* Must have a valid driver’s license.

* Must annually complete all training hours required by Outreach and/or accreditations.

Supervisory Responsibilities:
This job has no supervisory responsibilities at this time.

Work Environment:

The noise level in the work environment is usually moderate. Traveling in a high-crime/low-income area is often necessary. Local, regional and occasional long-distance travel required. The ability to work within a prison environment and with a prison population is required. Position is in Lackawanna County; an easy commute from Wilkes Barre, Hazleton, Bloomsburg, and Stroudsburg.

Physical Demands:

The employee must occasionally lift and/or move up to 25 pounds, occasionally transporting supplies/materials.
