ESSENTIAL DUTIES:
Model a service attitude towards others, takes ownership in solving problems and takes the initiative to make things better, is friendly and courteous to residents, patients, visitors, and co-workers
Ensure resident, patient, visitor, and co-worker safety by remaining aware of and reporting any new or existing safety hazards
Maintain good attendance record
Document all assessments, reassessments, progress notes, discharge summaries and other clinical work in the medical record per departmental policy and procedure
Provide resident, family and staff education pertaining to Physical Therapy modalities to ensure understanding and proficiency of care
Demonstrates good clinical judgment in all aspects of resident treatments in which trained to provide
Orients, instructs and directs work activities of Licensed Physical Therapy Assistants, Rehab Techs and Physical Therapy students
Reviews documentation written by the Licensed Physical Therapy Assistants
Screening residents to determine their potential benefit from physical therapy intervention
Develops, directs and/or participates in physical therapy programming for the residents
Attends in-services and other meetings as required on an ongoing basis
Maintains continuing education per state licensure requirements
Other duties as assigned by the Director of Rehab
MINIMUM QUALIFICATIONS:
Licensed Physical Therapist in the current state
Knowledge of therapy industry and federal, state and local government regulations
Demonstrated ability to organize, prioritize and complete multiple tasks in a dynamic environment
Demonstrated efficiency strategies for fast paced environment
Experience in SNF/LTC/IL/AL/OP setting or mature population rehabilitation and wellness needs
Experience with PDPM treatment and reimbursement policy
Basic computer skills, Outlook, Word, Excel
Excellent customer service skills
Genuine caring for and interest in the care of mature adults, persons with disabilities and hospice Patients
Ability to perform Essential Duties
Ability to perform work tasks within the Physical Demand Requirements
Ability to comply with the Resident Bill of Rights and the Employee Responsibilities
Ability to comply with organizational and departmental safety policies and procedures#ffl
