Job Title: Care Manager I, FEPI (First Episode Psychosis Initiative)

Location: Nashville, TN

JOB SUMMARY: Provide a Coordinated Specialty Care (CSC) model to consumers with severe and persistent mental illness experiencing first episode of psychosis due to schizophrenia or other severe mental illness.

RESPONSIBILITIES:

Continual involvement in the family system, school system and other systems in which the family may be involved (juvenile justice, other supportive agencies, etc).
Will provide Consumer care within the parameters of the Coordinated Specialty Care (CSC) model.
Organize and execute team case conferences, involving external treatment team members, one time monthly or as needed.
Participate in FEPI team on call rotation.
Participate in FEPI daily and weekly team meetings.
Participate in conferences and trainings as required.
Engage consumers and family unit to support the immediate and/or extended family.
Assist with referral and linkage to community resources and advocate for consumers whenever necessary.
Assist with follow up on missed or upcoming clinic, therapy, and IHC appointments.
Ensure consumer’s daily psychiatric, medical, and environmental needs are planned for and coordinated.
Ensure efficient transition of consumers when needed within the Agency and the community.
Meet minimum productivity standards as indicated in the Policies and Procedures.
Meet regularly with Program Manager for Supervision.
Attend team meetings as scheduled to facilitate communication and collaboration of consumers.
Facilitate education of consumer and/or involved collaterals to psychiatric diagnosis and appropriate treatment options.
Respond to urgent/crisis consumer needs as appropriate to position with assistance of supervisor, crisis services, and clinic staff by developing plans and interventions.
Provide wrap around services for children/adolescents by including all involved with the child (i.e., teachers, parents, grandparents, therapists, probation officers, etc.).
Consistently treat consumers and co-workers with respect.
Work within team environment Agency-wide.
Ensure timely and appropriate documentation as required by the Agency.
Maintain professional and ethical behavior at all times.
Develop and maintain community relations through prompt follow through with contacts.
Ability to adapt and integrate into different treatment team environments each month.
Possess excellent communication skills across teams.
Ability to quickly engage with various consumer populations.
Ability to quickly identify needs and work toward an immediate solution.
Ability to tailor their organizational system based upon their current consumer needs.
Possess a passion for the SPMI/SED population evidenced by delivering excellent service to each consumer they are covering.
