Your Impactful Role as an RN Case Manager:

The RN Case Manager is an integral part of the healthcare team at our Duluth office. Your responsibilities will include:

· Managing and Scheduling Visits: Taking charge of managing and scheduling your patient visits, you'll travel throughout the area to provide personalized 1:1 nursing services in the comfort of their homes. This hands-on approach ensures that our valued patients receive the highest level of care and attention.

· Interdisciplinary Collaboration: Your contribution to the interdisciplinary team is invaluable. By actively collaborating and utilizing your servant leadership and empathetic skills, you foster a supportive and cohesive environment, prioritizing the well-being of our patients and delivering the best possible care.

· Clinical Supervision and Inspiration: Your role involves providing clinical supervision to our caregivers, ensuring they follow the care plan diligently. Your ability to inspire them to deliver exceptional care in alignment with our core values of altruism and integrity makes a significant impact on the overall quality of care provided.

What You’ll Need to Excel:

To thrive in this position, you’ll need a valid Minnesota Registered Nurse license to practice and provide exceptional care in the Hibbing area, including the Iron Range.
