Job Description

Transition Age Youth Services serve current and former foster, probation placement and homeless youth ages 15-25. We provide an array of programs and services including counseling, skills building, a drop-in resource center, and subsidized supportive housing. Our mission is to support and empower youth in making a health transition into successful adulthood.

The TAY YHDP Case Manager is responsible for establishing a close working relationship with an assigned unhoused youth to provide quality comprehensive services. The Counselor will provide case management and mental health services, and act as liaison with outside agencies to assist youth in developing the necessary skills to live independently. Services will be delivered with dignity and respect for each individual client and a commitment to supporting youth reaching identified goals.

Responsibilities:

  • Advocate for youth and serve as an appropriate role model for youth.
  • Maintain all resident files in a confidential manner and in compliance with HUD and HIPAA standards.
  • Work in TAY drop-in Center daily and work with drop-in center team to maintain the safety and security of the facility in a trauma informed manner.
  • Participate in the effort to keep the facility clean, well maintained and running smoothly. This will include household duties such as shopping for supplies, laundry, and cleaning.
  • Follow up with weekly plans to assist and support client(s) in reaching identified goals.
  • Assist with connecting youth to resources and building skills to living independently for the first time (maintaining housing, employment, money management, social relationships, education, etc.)
  • Attend and participate in facilitation of weekly workshops, support groups, and other youth engagement activities.

Qualifications:

  • B.A. in related field such as sociology or psychology.
  • At least two years’ work experience with young adult populations, preferred.
  • Bilingual Spanish-speaking ability, preferred.
  • Strong knowledge of professional ethics required to work with vulnerable populations.
  • Strong professional writing and oral communication skills, as well as strong, culturally competent and respectful interpersonal communication skills.
  • Must have a vehicle, valid CA driver’s license, vehicle insurance, and good driving record to be covered by agency insurance.
  • Fingerprinting for licensing, criminal record clearance, and necessary physical exams are required

Wages & Benefits:
Our total compensation package includes a comprehensive medical and dental insurance plan for full time employees, 403(b) retirement plan, paid holiday, sick time, and 17 days of paid vacation, pro-rated for positions less than 40 hours a week.
