Job Description

The St. Luke's Transitional Care Center is looking for reliable LPN's (Licensed Practical Nurse) to care for our skilled nursing patients in a professional and considerate manner. You will work under the supervision of a Director of Nursing and Medical Director. LPN professionals must be qualified and compassionate. The ideal candidate will be experienced in providing basic nursing care to those who need it while remaining patient and calm.

Responsibilities

  • Follow established standards of nursing practice and implement facility policies and procedures.
  • Interpret existing policies and procedures to Resident Care Technicians, residents, families, and physicians.
  • Supervise and evaluate all direct resident care and initiate corrective action as necessary.
  • Receive support from the nurse you are relieving and record sufficient information to implement follow-up action as necessary.
  • Provide report to nurse coming on duty, including sufficient information for follow-up action if necessary.
  • Conduct resident rounds daily, reporting problems to the charge nurse or Director of Nursing, if necessary, and initiate correct action.
  • Identify resident problems, emergency situations, and respond appropriately within the Quality-of-Life Policies of Healthcare of Iowa.
  • Identify safety hazards and take appropriate action.
  • Follow, supervise, and evaluate the implementation of the Resident’s Rights Program.
  • Provide clinical supervision of Resident Care Technicians giving direct resident care.

Skills

  • Proven experience as LPN or similar role
  • Ability to undertake a variety of diverse care tasks and work in a multidisciplinary team
  • Familiarity with health, safety and sanitation standards and procedures
  • Understanding of confidentiality obligations and nursing best practices
  • Excellent knowledge of medical and hospital terminology
  • Excellent communication and interpersonal skills
  • Compassionate and able to handle stress
  • Successful completion of LPN license program is a must
