The Program Therapist in behavioral health inpatient units plays a crucial role in patient care. Responsibilities include conducting psychosocial assessments, developing treatment plans, and coordinating discharge needs. Essential functions involve discharge planning, conducting CBT-based group therapy, facilitating skills groups, and offering specialized clinical social work services. The role also includes crisis intervention, providing information and referrals, and collaborating with the interdisciplinary treatment team. Maintaining up-to-date discharge and referral resources and timely documentation is essential.

Formulate and conduct educational, therapeutic, and/or support groups.

Communicate effectively with patients, families, healthcare team members, and community providers regarding patient status and progress.

Interact and communicate with community agencies in the assessment of and service to patients.

Provide liaison and referral services to community agencies and appear in court when requested or subpoenaed.

Maintain confidentiality in compliance with professional ethics according to professional and facility standards.

Identify opportunities for the program and participate in Performance Improvement projects and activities.

Continue professional development in individual, family, and group therapy and maintain professional development in personnel files.

Assist in the orientation of new colleagues and supervising student interns.

With assistance from the Care Center, social workers, and nursing, identify all outside professionals currently working with the patient so that appropriate discharge plans can be completed and facilitate appropriate patterns of communication to minimize duplication.

Identifies needs of referral sources and/or professionals during the period of hospitalization.

Oversees the tasks of continued communication of treatment progress, discharge plans, and time of discharge to outside interested parties, as well as facilitating the transfer of required documents to the outside agents.

Interfaces with the Business Development Department to assist with the determination of customer satisfaction and problem identification and solutions.

Responsible for discharge plans including scheduling of appointments and communication with parents and significant family contacts.

Assists with chart audits and completion of medical records.

Inform patients and families on discharge planning options based on diagnoses, prognoses, resources, and preferences related to home care services.

Document findings, discharge plans, group and therapy notes, and actions taken on medical record according to departmental guidelines; prepare reports and maintain records as required.

Documentation must be made on all patient interactions within 24 hours.

Act as an educational resource and provide consultation to hospital personnel regarding the discharge planning process and applicable federal, state, and local regulations; identify benefits, implications, and limitations of continued care as appropriate.

Participate in multidisciplinary team meetings regarding the planning and implementation of patient care; facilitate communication and problem-solving related to discharge planning.

Perform a variety of clerical activities associated with the discharge planning procedures and the transfer and replacement of residents into facilities and community service agencies such as mental health programs, substance abuse programs, and outpatient programs.

Assist patients with transfer and discharge procedures by providing information and instructions regarding accommodations, finance, and insurance.

The Program Therapist is a critical role on the behavioral health inpatient units. The Program Therapist is responsible for completing the patient’s psychosocial history, master treatment plan and individual treatment plan, and assessing discharge needs. Important functions of this role include discharge planning with patient and collateral input; providing CBT-based group therapy; facilitating skills groups; and offering specialized clinical social work services for patients and families receiving care. Program Therapists may also be expected to provide crisis intervention, and information and referral as needed.

This role functions as a part of the interdisciplinary treatment team and prepares new admission patient summaries and regular patient updates as needed. The role also is responsible for maintaining current discharge and information/referral resources in the community, and communicates with these organizations in a timely manner to establish aftercare for patients. Documentation is to be completed according to timeframes established by state, federal and regulatory authorities.

Qualifications
Requirements:

Education: A Master's Degree from an accredited college or university in Social Work or a clinical-related mental health field is required.

Experience: A minimum of two (2) years experience in a psychiatric health care facility, with direct experience and strong skills in biopsychosocial assessment, treatment planning, treatment skills based on CBT methods, group therapy, and discharge planning. Crisis intervention skills are also required.

License: At a minimum, an Indiana State Social Work (LSW) or mental health counseling (LMHC-A) license is required; the LCSW or LMHC license is preferred.

Additional Requirements: CPR certification and management of aggressive behavior training within 30 days of employment prior to assisting in a restraining procedure.

Hendricks Behavioral Hospital is a state of art behavioral health and addictions hospital that opened in 2021 in the city of Plainfield, Indiana. HBH provides a full continuum of both inpatient and Intensive Outpatient Programs for adults with mental health and chemical dependency disorders. HBH provides both inpatient and Intensive Outpatient Programs for adolescents ages 13 to 17 with mental health and dual diagnosis disorders.
