As an LPN (Licensed Practical Nurse) your primary responsibility is providing high-quality skilled nursing care to patients in their home. This includes administering medications, monitoring patients' health, collaborating with healthcare professionals, and ensuring a safe and comfortable home environment for patients. You may also assist with activities of daily living, educate patients and their families on healthcare routines, and maintain accurate medical records. Adaptability, compassion, and effective communication are crucial for success in this role.


Key Responsibilities:


1. Skilled Nursing Care:

- Provide skilled nursing services based on individualized care plans.

- Perform assessments and implement appropriate nursing interventions.

- Monitor and evaluate patients' health status, communicating any changes to the healthcare team.


2. Medication Administration:

- Administer medications as prescribed, ensuring accuracy and adherence to protocols.

- Educate patients and their families on medication regimens and potential side effects.


3. Patient Monitoring:

- Regularly assess and document vital signs and other relevant health indicators.

- Monitor medical equipment and ensure its proper functioning.


4. Collaboration:

- Collaborate with physicians, therapists, and other healthcare professionals to coordinate comprehensive patient care.

- Communicate effectively with the healthcare team to provide updates on patient progress and any concerns.


5. Patient Education:

- Educate patients and their families on healthcare routines, disease management, and preventive measures.

- Provide guidance on maintaining a safe and supportive home environment.


6. Documentation:

- Maintain accurate and up-to-date medical records, including patient charts, care plans, and progress notes.


7. Activities of Daily Living (ADL) Assistance:

- Assist patients with activities of daily living, ensuring their comfort and promoting independence.


8. Emergency Response:

- Respond to medical emergencies promptly and efficiently, following established protocols.

Requirements:
  • Current LPN license in [state].
