A few details about the role:

• Oversight of case assignments, service plans, patient progress, and discharge planning.
• Obtain pre-authorizations for managed care inquiries and/or admissions or work with hospital case management to obtain.
• Review documentation daily to determine any opportunities to either:
  • Improve documentation.
  • Request an elevation in level of care if appropriate.
• Develop, execute, and oversee process to efficiently communicate with the IDT and patient when skilled care will end and overseeing the execution of denial notices provided by the managed care entity.
• Oversight of collaboration with patients, families, physicians, nurses, and other member of the IDT to ensure high quality care.
• Oversee the IDT in acting as the patient's advocate related to patient satisfaction, insurance coverage and financial assistance.
• Promote quality and cost-effective interventions and outcomes.
• Coordinate with third party payors on a regular basis
• Assist with payer appeals.

And here's what you need to apply:

• Graduate of an accredited school of nursing with BSN
• Current Nursing License, RN preferred (compact license preferred)
• CCM Certification preferred.
• 3-5 years clinical experience, preferably in case management or related field
