Licensed Practical Nurse Manager

68,000.00- 70,000.00

We are looking for a compassionate LPN to join our growing team! We are a family-owned company. The right candidate has a can-do attitude, excellent interpersonal skills, and a current state LPN license. In this role, you will be providing medical support to our clients while ensuring a safe environment for them.

Responsibilities

  • Assess residents in home, hospital, rehab, and other settings to determine initial care plans and appropriateness for move-in
  • Complete re-assessments at 30 and 90 days, as well as when there are significant changes and conduct care conferences with residents and families to discuss care changes.
  • Manage all Resident Care Staff, including completing education, discipline, and performance reviews.
  • Communicate with ancillary staff and coordinate continuation of care services.
  • Oversee the Infection Prevention and Control program and lead quarterly meetings.
  • Oversee Incident Management and Quality Review Program
  • Manage all aspects of resident care to ensure that residents are well cared for and treated with dignity and respect.

Qualifications

  • 1 year experience as an LPN in a home care setting
  • Current state license as an LPN/RN
  • Proof of current CPR certification
  • Ability to work flexible schedule.
  • Excellent interpersonal skills
  • Valid driver’s license and reliable transportation
