We cannot guarantee specific unit assignments, shifts, schedules, or requested days off at this time. We also prefer that LPNs have at least one year of acute psychiatric nursing experience.

· 10 panel Drug Screening

· OIG Exclusions Database Search

· APS/CPS Form

· Recent Vaccinations-within a 5 year time frame (Flu, 2 PPDs, MMR, Varicella, HEP B series or titer, Tdap-must have proof of all)

· Covid Vaccine (if received)

· CPR (American Heart Association-BLS Only-Online Courses Not Accepted)

· Physical/Physician Statement
