Role: Case Manager - Community Health Reports to: Manager of Vail Connect and A.O.S FLSA Status: Exempt

What will you do?

The Systems Navigator/Community Health Worker position provides supportive services to people with complex needs to achieve safety and stability in the community. Individuals may be facing substance use disorders, chronic medical conditions, frequent hospitalizations, and lack of stable housing. This position works alongside other navigators, CHW’s, and Registered Nurse as part of an integrated care team with the goal of improving overall wellbeing of individuals served. This program addresses their many needs in ensuring continuity of care, referrals to needed resources, coordination with external partners, housing support, and connecting to long term support while utilizing a person-center approach to care. This position is in a unique program that partners with managed care organizations to support and connect their high-risk individuals to long term support.

To be considered for this opportunity you will bring your skills and talent, with a BA or BS human services or related area; BSW you must have a valid and current Social Work License or Community Health certificate. Further your valid driver’s license, and adequate insurance coverage, and access to an automobile.

Salary Range: $50,000+ based on experience
