We are looking for a Home Health Nurses (RNs and LPNs) to join our fast-growing team. Become part of a notable company and help us achieve our goals!

Shifts: Saturday and Sunday; 6a-6p

LPN $25-$35

RN $35-$45

POSITION SUMMARY:
Our RNs and LPNs provide direct patient care to our patients under the supervision of a physician. Responsibilities include following start of care plans, providing treatment, discharges, wound care, medication administration, IV, Tube feeding and working collaboratively with a clinical team to achieve positive patient care outcomes. You will be responsible for educating patients and their families on prevention and healthy habits. Ideally, as a well-trained professional, provide care with little supervision, be compassionate, and follow health and safety guidelines. Our goal is to promote the patient’s well-being by providing high-quality nursing care. 

ESSESNTIAL FUNCTIONS

· Submit regular patient care progress reports to attending physician and clinical staff.

· Consult & develop treatment plans based on physician's orders and initial patient assessment.
· Provide nursing care and written care plans.
· Participate in and monitor patient and family care compliance activities.
· Professionally manage and coordinate the plan of care
· Complete clinical and medical documentation on all patients.
· Monitor patient’s condition and assess their needs, to provide the best possible care and advice
· Observe and interpret the patient’s symptoms and communicate them to the physicians
· Should have experience with the following: IV experience, pediatrics/young adults, Trachs, G-Tubes, wound care, vent experience preferable, strong clinical experience,
· Must have critical thinking skills, strong clinical background
· Provide immediate medical care in emergencies
· Inspect the facilities and act to maintain excellent hygiene and safety (decontaminating equipment, sanitizing surfaces, preparing beds, etc.)
· Foster, a supportive and compassionate environment to care for patients and their families

REQUIRED EXPERIENCE:
· Minimum of Associate degree/diploma (or higher) in Nursing.
· Current and unrestricted Florida RN/LPN license.
· Current CPR/BLS certification.
· Minimum of 1 year RN or LPN experience in an institutional setting.
(Acute Care/ Rehab / Hospital /Home Health /Hospice)
· Excellent working knowledge of nursing care methods and procedures
· Team player with excellent communication and interpersonal skills
· Be dependable, reliable, responsible, patient, and compassionate
· Outstanding organizational and multi-tasking skills
· Excellent problem-solving skills
