Job Description
Liberty Home Care is looking for a motivated Registered Nurse to join our growing home care nursing team full-time.

Duties & Responsibilities
  • Provide skilled nursing services to agency patients.
  • Provide assessment of newly referred patients, assess health needs and eligibility for home care.
  • Provide patient and family teaching prior to discharge and on an on-going basis.
  • Develop and implement care plans in conjunction with patient’s authorized medical practitioner.
  • Reassess patient needs for services on a continual basis and discuss any changes in the plan of treatment with the patient physician/family.
  • Prepare written documentation and summaries of the patient’s progress as needed for the continuation of home health services.
  • Plan, facilitate, and document patient’s discharge.
  • Provide supervision and training of PCA/HHA staff
  • Assist with the on-call rotation.
Qualifications
  • NYS Licensed Registered Professional Nurse.
  • Minimum of Associate's Degree required.
  • One year Nursing experience required.
  • One year home care experience a plus.
