Social Worker / Case Manager
The Social Worker / Case Manager is involved in counselling and assisting clients to meet personal, social, health, and economic needs.
This position can be in Milford or Georgetown Delaware
Hours: Mon - Fri 8am to 4:30pm
Rate of Pay: $20 an hour
Job Responsibilities:
- Applying person-centered planning approach, makes home and/or field visits to interview client, family and/or professionals to create a plan of care that supports individuals in the least restrictive setting.
- Assist participants with the Diamon State Health Plan Plus application process. Assist with gathering documents & monitoring the process until a determination is made.
- Makes referrals and assists clients in applying for a variety of services and/or programs.
- Maintains a caseload, determines initial and ongoing eligibility for services under the Community Services Program.
- Complete activities by the due date.
- Create activities in companies Aging & Disability database as outlined in Job Aid.
- Respond to routine calls/emails within 2 business days of contact.
- Utilize companies Aging & Disability database, Outlook Calendar, Tracking Assessing Planning (TAP), ASSIST Worker Web (AWW), Vital Statistics, and Delaware Medicaid Enterprise System, (DMES), for casework activity.
- Check New Referral saved search daily to retrieve referral.
- Contact participant to complete a application over the phone. Monitor application process from start to decision.
Job Requirements:
- Must have 6 months or more of Case Management experience.
- Must have narrative writing skills.
- Must be proficient in Microsoft Suites, Word and Excel.
- Must have a valid driver license.
