Description

Facilitates the transition of high risk inpatients to the home environment or appropriate post-acute care provider. Identifies resource and educational needs and develops individualized care plans accordingly. Collaborates with hospital Care Management, hospitalists, community physicians and other providers of the Integrated Delivery System to ensure smooth transition to the most suitable level of care. Promotes patient education and self-care management in order to reduce hospital readmission rates.

Required qualifications:

  • Coursework/Training: RN from an accredited school of Nursing.
  • Upon hire: California Registered Nurse License
  • 2 years Clinical acute experience.

Preferred qualifications:

  • Bachelor's Degree Nursing. Required to obtain within 3 years of hire date
  • Upon hire: Case Management certification (required within 1 year of hire)
  • 3 years Experience in performing community nursing, case management discharge planning or related experience.
