Home Health Aide/Care Giver:

OakMed Home Nursing & Rehabilitation is looking for passionate, dedicated Home Health Aides/Care Givers to join our team in the Grand Rapids Area. The perfect candidate will be professional and compassionate to provide meaningful, one-on-one care to our patients in their home. The availability for this position is 12 hour shifts on Saturdays and Sundays with both Days and Overnights available!

As a Home Health Aide/Care Giver, you will be responsible for the day-to-day care of our patients in their homes, and will be working directly with our nursing department to administer the proper care needed for each patient.

Requirements:

· High School Diploma or Equivalent (Required)

· Driver’s License (Required)

· Personal Transportation (Required)

· Proof of Vehicle Registration (Required)

· Active Auto Insurance Policy (Required)

· Must Pass a Criminal Background Check (Required)

· Must be eligible to work in the United States (Required)

· Experience as a Home Health Aide or Care Giver (Preferred)
