Job Description
Essential Duties and Responsibilities
The Behavioral Health Clinician will provide mental health therapy services within a primary care-based clinic. This position requires the formulation of an individualized treatment plan which will be used in conjunction with the entire primary care team to develop a holistic care strategy for each client.
Perform related duties as assigned by supervisor
Maintain compliance with all company policies and procedures
Education, Licensure and Experience Requirements:
Education: Master’s degree in Social Work (MSW) Psychology, Counseling or Doctoral Degree in Psychology from an accredited college/university
Required Licensure: LMSW (Licensed Master Social Worker), LLMSW (Limited Licensed Master Social Worker) or LP (Licensed Psychologist)
Minimum Required Experience: Must have experience in working effectively with a diverse population of consumers/clients with mental illness, or other psychosocial concerns.
Required Skills:
Proficiency in MS Office (Word Excel and Outlook)
Proficiency in PC and general office equipment (Fax, Copier, scanner)
Ability to maintain good records and be able to complete reports via electronic record (EMR) or ECO as needed
Excellent written and verbal communication and documentation skills
Job Knowledge and Comprehension
Person Centered Planning includes completion of the person-centered planning document that provides an overview of the person and details his/her hopes, dreams and desires.
Individual Plan of Service includes completion of treatment goals, measurable objectives, assigned staff and applicable treatment codes.
Consent and Enrollment includes completion of required consent and enrollment documentation.
