Job Description

The Social Services Representative provides general assistance to our patients; completing processes and applications to receive benefits through the local resources and the government assisted programs.

Duties and Responsibilities

  • Assist patients with the completion of the applications for SNAP, Medicaid, Medicare Saving Program, or any other Government benefit program, based on the information they provide and with the submission of verifications and general information regarding the application process.

  • Assist patients to make calls related to benefits and provide printer, fax, and copy machine for application related documents.

  • Help patients and visitors activate or request their Electronic Benefits Transfer card (EBT).

  • Assist patients with the application of Medicare, Supplemental Security Income (SSI), social security card, change of status, or other benefit related to Social Security Administration (SSA).

  • Assist patients in completing the naturalization process application and follow up until completion.

  • Complete American Passport application and make the appointment to the corresponding office.

  • Assist patients with the application of government cells phones, affordable high-speed low-cost internet, Home Energy Assistance Program (LIHEAP), housing, Section 8, or any other public assistance benefit.

  • Assist patients with the application for special transportation services, bus pass application or disabled person parking permit.

  • Participate in monthly department meetings and service training to maintain the standards certifications.

  • Handle any complaints from patients and family members as appropriate.

  • Complete all processes and handle all confidential information in an honest, ethical, and professional manner.

  • Ensure that all HIPAA standards are met.

  • Perform other duties as assigned. 

Qualifications / Education / Licenses

  • High school diploma, equivalent, or relevant experience required.

  • Previous experience with any government assistant programs and immigration services.

  • Bilingual in English and Spanish preferred; must be able to read, write and speak English.

  • 1-2 years of experience in customer service environment preferred.

  • Basic computer knowledge including Microsoft Office, internet, document with electronic health records.
