Job Description

Provides quality and compassionate direct clinical primary care, urgent care, and patient outreach/education to patients of all ages in outpatient community health centers. Diagnoses, educates, and treats patients to ensure that they have the best possible care. Makes independent clinical decisions in consultation with an integrated care team, participates in the development of health programs based on the needs of our Tribal Members and other patients, and helps to lead and mento support staff.


Work Schedule:

Choice 1: Monday - Friday; 8:00 AM - 5:00 PM; 1 Hour Lunch Break

Choice 2: Monday - Thursday or Tuesday - Friday; 7:30 AM - 6:00 PM; 30 Minute Lunch Break


Job Functions:

include, but not limited to, the following:

  • Provides direct, efficient, and quality health care services to patients-including evaluation for additional services and follow up care. Develops and assures implementation and ongoing status of recommended treatment.
  • Provides evidence-based care with consideration of nationally recognized and FourPoints Health internal clinical guidelines.
  • Recommends, participates in, and works to ensure the success of efforts to improve cost effectiveness and quality of care provided to patients.
  • Participates in clinical group meetings to evaluate patient care efficiency and quality improvement efforts
  • Offers clinical consultation and arranges referrals to internal/external providers and clinical staff. Accepts patient referrals from other physicians.
  • Performs minor surgeries.
  • Manages chronic health problems through coordination with internal and external service providers and assures patients are tracked and monitored for continuity of care through affective case management.
  • Participates in academic programs (e.g. medical education and research) as requested.
  • Provides appropriate documentation that meets insurance company requirements; works with the billing office to resolve all denials in a timely manner.
  • Participates in inpatient and outpatient consultation for specialty, which may include on call schedules.
  • Participates in on call schedules; divided up amongst other providers.
  • Provides supervision to advanced practice providers.
  • Completes entry of patient health data/charges into the electronic medical records system (EMR) database in a timely manner.
  • Explains procedures or prescribed treatment to patients and includes them in developing care plans.
  • Responds to emails and other organizational communication messaging in a timely manner.
  • Provides clinical mentorship and supervisor of Nurse Practitioner and Physician Assistant colleagues assigned.
  • Attends all staff meetings and all other meetings as assigned.
  • Completes assigned training courses in MedTrainer.
  • Meets with Clinic Manager and Clinical Director as needed for logistical issues or concerns.
  • Orders appropriate blood work and other laboratory procedures, performs CLIA waived laboratory tests.
  • Reviews radiologic and laboratory tests results to identify and communicate abnormal findings.
  • Completes patient encounter notes into EMR chart during encounter and documents inpatient or outpatient encounter in the EMR.
  • Prescribes and administers medications.
  • Completes follow-up care as appropriate via telephone or telemedicine.
  • Completes all inpatient and outpatient medical assessments, clinical documentation, medical orders, laboratory and consult reviews, plans of care, treatment reviews, and summaries in a timely fashion.
  • Provides education to patients, as it relates to their medical condition.
  • Recommends universal precautions when ordering any invasive procedure for diagnostic or routine studies.
  • Provides services in accordance with the Centers for Disease Control infection guidelines, Federal, State, and local laws, and the Quality Improvement Program standards.
  • Assumes responsibility and accountability for his/her individual judgments and actions.
  • Maintains and submits documentation of current medical licensure, DEA registration, and any relevant certifications in a timely manner.
  • Supports the FourPoints Health Patient Outreach Program by submitting patient referrals to the Patient Care Coordinator for follow-up and support.
  • Ensures that the staff are trained and evaluated on their knowledge of and adherence to compliance policies and procedures specific to their jobs.
  • Participates in community health promotion/wellness events.
  • Always maintains confidentiality and impartiality.
  • Other duties as assigned to support the efficient operations of the Tribe.

Requirements

General Requirements:
  • Must be insurable through the Tribe’s vehicle insurance required.
  • Must pass a criminal and fingerprint background check according to P.L. 101-630 standards required.
  • Must pass a pre-employment alcohol/drug screening required.
  • Must pass a physical examination prior to or contemporaneous with hiring required.
  • Must successfully pass all credentialing requirements required.
Education and Experience:
  • Doctorate degree in Medicine (MD) or Osteopathy (DO) from an accredited medical school required
  • Medical residency training from an accredited educational institution or school required.
  • Experience with the Indian Health Services health care system preferred.
  • Experience working with AAAHC, JCAHO and Tribal Public Health Accreditation preferred.
License & Certification:
  • Must possess, maintain, and provide proof of a valid Driver’s License required.
  • Must possess, maintain, and provide proof of a valid Utah Board-Certified Physician MD/DO License required.
  • Must possess, maintain, and provide proof of a valid DEA License required.
  • Must obtain, maintain, and provide proof of a valid First Aid, CPR and BLS Certified (Red Cross or American Health Association) within three (3) months of hire.
