We are hiring for a Hospice RN/Case Manager for Portland, Oregon market who wants to be part of a growing and collaborative team. We believe in honestly, integrity and a dedication to our patients dignity in end of life care.
Our Ideal candidate must be capable of compassionate communication and have the observation skills with high ethical standards and knowledge to alert the team and doctors about changes in patient conditions. Dealing in a hospice setting, it is important to be resilient and sympathetic, along with having emotional and physical stability to deal with severe illness and death.
Responsibilities
  • Monitor patients condition and assess their needs to provide the best possible care
  • Observe and interpret patients symptoms and communicate them to Director of Nursing and EMR system
  • Collaborate with Medical Director, social worker, chaplain and team regarding patients status and needs.
  • Perform routine procedures (blood pressure measurements, administering injections etc.) and fill in patients charts
  • Monitor patients medication needs and provide insight to medical director and Director of Nursing
  • Foster a supportive and compassionate environment to care for patients and their families
  • Expand knowledge and capabilities by attending educational workshops, conferences, online training's, etc
  • Must be flexible and open to weekend admits if necessary.
Skills
  • Proven experience as a registered nurse
  • Excellent knowledge of nursing care methods and procedures
  • Hospice experience and knowledge
  • In-depth knowledge of health and safety guidelines and procedures (sanitation, decontamination etc.) and willingness to follow them at all times
  • A team player with excellent communication and interpersonal skills
  • Responsible and compassionate
  • Outstanding organizational and multi-tasking skills
  • Patient with excellent problem-solving skills
  • BSN, MSN or other diploma from a nursing program
  • Valid OR nursing license

Compensation is based on experience and will be discussed during interview.
