Primary Responsibilities:

  • Assess, plan and implement care strategies that are individualized by patient and directed toward the most appropriate, lease restrictive level of care
  • Identify, initiate, and follow up on referrals for community based programs and providers; including financial, psychosocial, community and state supportive services
  • Manage the care plan throughout the continuum of care as a single point of contact
  • Communicate with all stakeholders the required health-related information to ensure quality coordinated care and services are provided expeditiously to all members
  • Advocate for patients and families as needed to ensure the patient's needs and choices are fully represented and supported by the health care team

The schedule for this position is  Monday - Friday, 8am - 5pm.  This is a field based position visiting our members in their homes, community based housing, and long-term, skilled and acute care facilities in the Washington, DC area. You'll need to be flexible, adaptable and, above all, patient in all types of situations.

This is high volume, customer service environment. You'll need to be efficient, productive and thorough dealing with our members over the phone. Solid computer and software navigation skills are critical. You should also be strongly patient-focused and adaptable to changes.

Required Qualifications:  

  • Current, unrestricted RN license in Washington, DC
  • 3+ years of experience in a hospital, acute care or direct care setting
  • Demonstrated ability to type and have the ability to navigate a Windows based environment
  • Access to reliable transportation
  • Ability to work in a field based capacity in Washington, DC
  • Reside in Washington DC

Preferred Qualifications:  

  • BSN
  • Certified Case Manager (CCM)
  • Background in managed care
  • Case management experience
  • Experience or exposure to discharge planning
  • Field based experience going into homes
