Overview:
EIHAB Human Services is hosting a Walk-In Hiring Event on Thursdays for individuals interested in joining our dedicated team. We are seeking compassionate individuals with a passion for providing care and support to those in need.
The Event will be held at 615 Merrick Avenue, Fresh Meadows 11554.

Direct Support Professional:
Responsible for the security, safety, and well-being of the individuals attending the Day Habilitation program according to his/her specific needs and interests, with the purpose of increasing the quality of life for each person by promoting their respective level of independence.
- Implement goals, training, and community inclusion activities as per the individual's Day Habilitation Plan.
- Work with individuals in groups or individually as assigned.
- Document individuals’ progress on a daily basis, or as required.
- Transport individuals as necessary to programs, activities, appointments, recreational functions, etc. as scheduled or requested by supervisors.
- Ensure wheelchair-bound individuals are safely and securely strapped and unstrapped in agency vehicles when traveling.
- Assist individuals in preparing and eating meals.
Receptionist:
The successful candidate will be responsible for the receptionist desk, carrying out general administration duties, and will be required to assist with the day-to-day running of the program. We need a lively, enthusiastic person who can copy under pressure to work in our busy accommodation office.
- Responsible for the coverage of receptionist duties of other facilities as requested by management
- Responsible for carrying out the day-to-day administration of the front office, including answering telephones, and emails, sorting and distributing mail and faxes.
- Assist the Director with the day-to-day running of the programs at the assigned facility
- Provide administrative support to the program supervisors (this means that she will be responsible for the typing of the reports provided by the supervisors)
- Ensure that the facility is kept clean and in order at all times
- Responsible for the order of supplies
- Ensure that the front office functions are running effectively daily
- Report any issue regarding occurrences in the facility to the Director
- Work on other projects as assigned by management
Community Habilitation Specialist:
The community habilitation specialist will work directly with individuals in program activities, both in their homes and in the community. Assist individuals (service recipients) in skills development by implementing individual-specific and general programs. The Community Habilitation Specialist will have the ability to interact with individuals and staff professionally and ethically, understand and implement service plans, and Staff Action Plans as written, and complete all required documentation clearly, legibly, and promptly
- Please participate in the life of the individual in their home to foster a person-centered atmosphere conducive to the growth of the individual and towards their maximum independence in all areas of daily living. Commit to person-centered support and honor the personality, preferences, culture, and strengths of the individuals. Promote and protect the health, safety, and emotional well-being of the individuals
- Assist in planning and implementation of the program.
- Maintain daily Community-Habilitation bi-weekly progress reports, datasheets, timesheets, and other required documentation. Community Habilitation Specialist will document each training session in the appropriate databases and report the progress and barriers with each client.
- Attend and participate in staff mandatory and In-service training and meetings as required. Community Habilitation Specialists must know and protect individual rights.
- Maintain contact with Program Supervisors and individuals and family about changes in schedules, lateness, absences, and other issues that will affect working with the individual.
- Communicate relevant information to Program Supervisors verbally and in writing, i.e., daily notes, incident reports, etc.
- Complete documentation electronically as required. Follow up with electronic visit verification (EVV) on assigned dates.
- Follow all specified policies and procedures, for managing behaviors, emergencies, and reporting incidents
