Resident Care Coordinator
Type: Part-TimeShift: Various schedule, availability to cover shifts/be on-call on a regular basis is requiredWage: $19-22/hour DOELocation: Avamere at Hermiston - 980 W. Highland Ave., Hermiston, OR 97838

The primary responsibility of this position is to assist in the management of clinical staff and proper documentation that supports resident care.

Responsibilities:
  • Document information pertaining to changes in residents’ status, needs and appointments.
  • Ensure proper documentation of all care plans, nurses notes and all other resident correspondence.
  • Strong experienced as a Medication Aide.
  • Provide orientation and on the job training to Caregivers and Medication Aides.
  • Assist with daily living activities (i.e., bathing, dressing, and eating).
  • Measure, record and assist with wellness clinics.
  • Conduct weekly, monthly, and quarterly audits.
  • Manage scheduling of clinical staff.
  • Maintain confidentiality of all resident care in accordance with HIPAA guidelines

Qualifications:
  • High-School diploma or equivalent.
  • Certified Nursing Assistant preferred.
  • Must have First Aid & CPR Certification upon hire
  • Must have a Food Handlers Card.
  • Must have at least 1 yr. of strong experience as a Caregiver or Medication Aide in an Assisted Living Community
  • Ability to provide proof of COVID-19 vaccination preferred, but not required
