At CMCS (Cardinal McCloskey Community Services), our mission is to protect, empower and promote independence for at-risk children and families and those with developmental disabilities through quality community-based services.

As Community Service Base Provider, (CBSP) you will be responsible for direct service provision to children and their caregivers enrolled in the Children & Family Treatment Supports and Services (CFTSS) and Home & Community Based Services (HCBS) programs. The CBSP will receive a combined caseload that may include Community Psychiatric Supports and Treatment Services- Components 4-6 (CPST); Psychosocial Rehabilitation (PSR); Caregiver Family Advocacy and Supports & Services (CFAS) and Pre-vocational; The CBSP will be responsible for the provision of goal-driven direct services of all cases and all prescribed service hours on a monthly basis. The PD CBSP will also be responsible for the successful completion of all Treatment Plans, safety plans and service-related service notes.

The successful candidate for the CBSP position will be a highly organized, independent worker who is able to track/monitor all service hours for a diverse caseload. This candidate must also be able to work using a multi-disciplinary approach and drive services from goals in treatment plan/Plan of Care.

Requirements:

• Bachelor's degree and one year of applicable experience in children's mental health, and/or foster care /child welfare/juvenile justice system.
• Understanding of mental health diagnoses, symptomatic behaviors and emotional response to trauma.
• Flexible schedule for direct service provision, which includes evenings and weekends as needed.
• Strong communication skills (oral and written)
• Bi- Lingual Required ( English/Spanish)

Additional requirements:

• The CBSP is expected to provide 100% of their assigned client's weekly services.
• The CBSP is expected to commit to between 35 and 45 hours of service provision per month.
• Successful candidate will be responsible for all service-related tasks including the completion and submission of notes within 48-72 hours of service rendition therefore, the CBSP must have consistent access to a computer/laptop, internet and flexibility to attend meetings and communicate with Managed Care Organizations.
• Attend all mandated trainings including, but not limited to: Mandated Reporter, Evolv, Service-based trainings and other best-practices trainings.
• Attend monthly Clinical Supervision and individual CBSP supervision with direct supervisor.
• Check emails daily and respond to emails from Care Managers, CBSP Supervisor and other team members within 24-48 hours.
