Company Summary:

AuSable Valley Community Mental Health Authority provides quality prevention, education, and mental health services, in a fiscally responsible manner, in Iosco, Ogemaw, and Oscoda Counties, which are aimed at improving the health and welfare of persons served, promote greater independence, and improve the quality of life for people in these counties with developmental disabilities, mental illness or substance use disorders.

Case Manager 

The Case Manager is responsible for advocacy, coordination and linkage of services to provide continued care and to avoid rigidity, fragmentation, misuse and/or inaccessibility of services.  Must possess the ability to link the consumer to services and coordinate the various services to achieve successful outcomes.  Provides services to assist the consumer in gaining access to needed medical, social, educational, therapeutic and other support services.  Major functions and core elements of the role includes assessment, service plan development, linking and coordination of services, reassessment/follow-up and monitoring of services and advocacy.

Requirements:

  • Bachelor’s Degree (B.A.) in human services field
  • One year experience in the mental health field
  • Certified as a QMHP, QIDP, and/or CMHP and/or be limited or fully licensed in the State of Michigan in their relative field
  • Emergency Services On Call Responsibilities.
