Employment Specialist 099 (21757) Description Works as part of a multi-disciplinary team grounded in the housing first approach, and coupled with an integrated service model to assist families in obtaining and retaining housing in Somerset, Hunterdon, Warren and Mercer County. Responsible for establishing a trusting relationship with families and working with them to identify their service goals. Holistic planning approach to assist families with connecting to, navigating and coordinating needed health and social services. Requirements Bachelor’s Degree in related field required with 2 years related experience. Case management/documentation; rotating 24/7 coverage required. Case conference with CP&P and other providers. Requires a flexible schedule including availability on evenings. Driver’s license needed with an acceptable driving history. Remote work possible after 3-month probationary period.
