Job Description - DIRECT CoordinatorOVERVIEW

Diversion Hub is a comprehensive one stop network designed to assist those who are actively entangled in the criminal legal system by providing resource coordination, system navigation, and connection to multiple on-site community agencies to increase access to services. The Diversion Referral and Connection Team (DIRECT) surveys incarcerated individuals within the Oklahoma County Detention Center and connects them to programs to get them out of custody. The goal is early identification of those who qualify for bond and diversion programs therefore significantly reducing the average amount of time an individual will spend in the custody of the Oklahoma County Detention Center. The DIRECT program has staff embedded in both the jail and the courthouse, focusing on surveying and coordinating the referrals and release of individuals. The DIRECT Coordinator is embedded within the Oklahoma County courthouse and will utilize DIRECT surveys to coordinate releases.

RESPONSIBILITIES

The DIRECT Coordinator will work to efficiently utilize DIRECT surveys to connect in-custody individuals with potential bond options and diversion programs while also working on discharge planning for individuals being released to the Diversion Hub from the Oklahoma County Detention Center. The responsibilities of the DIRECT Coordinator are as follows:

  • Demonstrating the values expected within the program and promoting the mission of the organization.
  • Evaluating DIRECT survey results to screen for program eligibility and making timely and concise referrals to any pertinent legal system entities or community agencies.
  • Conducting timely and thorough follow-ups on all referrals made to confirm if referrals have been approved or denied. Furthermore, utilizing any other referral options after a denial occurs.
  • Analyzing the needs of individuals scheduled to be released to Diversion Hub and creating a discharge plan focused on reducing initial barriers upon release. These discharge plans may include: an immediate connection to stable housing, benefit navigation, access to food and clothing, connection to treatment, or any other service referral that may be relevant.
  • Preparing necessary documentation for Diversion Hub staff to assist with quicker connections to emergency needs and allowing assigned case managers to have a full understanding of required action steps moving forward.
  • Maintaining database to track pertinent information relating to the client, including referrals, outcomes, meeting notes, and any other relevant information.
  • Maintaining constant communication with the public defender's office and Diversion Hub staff to ensure all individuals' legal requirements are met upon release.
  • Collaborating with all pre-trial bond and diversion programs to maintain the most up-to-date understanding of all necessary processes.
  • Serving as an ongoing liaison to partner agencies, attorneys, law enforcement, and all referring sources.
  • Maintaining adequate communication channels with Justice Navigators, onsite service navigators, and clients.
  • Always adhering to agency code of ethics and professional standards including maintaining strict client confidentiality.
  • Attending all staff development, training, and meetings as required.
  • Any other duties within the scope, spirit and purpose of the job.
EDUCATION AND EXPERIENCE

1 to 3+ years of experience in social work, communication, criminal justice, political science or similar human service area. Experience in case management preferred.

ADDITIONAL JOB REQUIREMENTS

Intermediate computer, mobile device and internet skills with knowledge of software programs including but not limited to:

PHYSICAL DEMANDS

While performing the duties of this job, the employee must be able to sit over long periods of time and be able to talk, hear, sit, stand, walk; use hands to finger, handle or feel objects, tools, or controls; reach with hands and arms; and stoop, kneel, crouch, or crawl climb stairs, lift and or move items of up to 25 pounds. Specific vision abilities required by this job include close vision, distance vision, and the ability to adjust focus. Must have excellent command of English language and grammar, both verbal and written. Must be able to manually operate and use a computer. Must be able to clearly hear and understand telephone conversations.
