We are seeking highly organized, proactive, compassionate individuals to join our team as Member CARE Advocate to support our members, their families, and our clinical team. 

As a Member CARE Advocate, you will assist in identifying challenges being experienced by our members and work directly with those individuals, as well as a myriad of local and national resources, to elevate or eliminate those instances. We partner with our Gloria Gates CARE healthcare team to address the needs of members and their families, assisting with member-facing duties as needed.  Success in this initiative necessitates the development of a profound level of trust with our members, understanding their healthcare needs and engaging them on how we can best support them and their families.

KEY RESPONSIBILIES:
  • Collaborate closely with internal team members and interdisciplinary stakeholders to ensure seamless execution of projects and alignment with organizational goals.
  • Continual development and fostering of trusting relationships with members and their families.
  • Compassionate, thorough guidance and assistance to members and their families relating to all components of their healthcare and socioeconomic needs.
  • Responsible for working on comprehensive CARE plans for members, ensuring their needs are met efficiently and effectively.
  • Play a pivotal role in spearheading the development and coordination of healthcare-related events to foster engagement and education within the community.
  • Maintain monthly contact with the member to address identified Social Determinants of Health (SDOH) barriers, develop plans to alleviate concerns, and provide general support, while also collaborating with member support person(s) to achieve mutual goals.
  • Ensure accurate documentation of member information and actions in the medical record ensuring transparency and continuity of care, facilitating effective communication among healthcare providers, and supporting informed decision-making for the member's treatment.
  • Facilitate the provision of referrals for services to community and national agencies as deemed appropriate, ensuring members have access to the necessary support and resources for their holistic well-being.

EXPECTATIONS:
  • Demonstrate and promote the values of respect, honesty, and dignity for the patient, families, and the healthcare team.
  • Ability to work independently and within a team.
  • Basic computer skills.
  • Proficiency in Microsoft office software and communication tools.
  • Strong interpersonal and communication skills.
  • Ability to multitask and prioritize effectively in a fast-paced environment.
  • Proficiency in electronic health records is preferred to efficiently navigate and utilize vital patient information, facilitating seamless coordination of care and accurate documentation across healthcare settings.
  • Valid Driver’s License and reliable transportation.
  • Willingness to travel in the community up to 10%
  • Maintain confidentiality and protect the practice by abiding by all laws and principles related to confidentiality; keep information concerning practice operations, members, their families, and staff confidential. 
  • Successful completion of background check and drug screen.
  • Maintain compliance with all Gloria Gates CARE standards, including Conduct, Compliance, and all other policies and procedures. 
 
EDUCATION AND EXPERIENCE:
  • Associate or bachelor’s degree in healthcare, social work, or related medical field.
  • 2 plus years of experience in a healthcare or case management role
