JOB DEFINITION:

  • Licensed Clinician to provide evaluations, mental health assessments, and if indicated, short-term crisis therapy for students grades K-12. The Licensed Clinician will also ensure linkage to ongoing community treatment coordinated through the student/family's insurance coverage. This position will also act as a liaison between the providers, families and district to aid in improved educational outcomes for students.

DUTIES AND RESPONSIBILITIES:

  • The Clinical Services Specialist will spend 50% of their time with the students and/or school staff/personnel and 50% of their time with families and community providers completing linkages. CN Guidance and the appropriate school district staff will work together to determine the most effective schedule.
  • Gather sufficient information for each student to make a diagnostic assessment necessary to determine referrals for mental health supports in the community
  • Care coordination-helping students/families identify their full array of service needs (beyond behavioral health) and providers across the community who can assist in meeting those needs
  • Referral and "warm hand-off" (meaning that the liaison referring helps the student/family to directly and fully connect with the agreed-upon specialist[s]). Such warm hand-offs will be to community providers with family insurance as primary payor.
  • Implement appropriate crisis services when needed.
  • Regularly collaborate with school mental health staff the progress of students' treatment needs and linkage to referrals.
  • Exchange information with other agencies that benefits the student if needed
  • Knowledge of community resources that are relevant to youth and families
  • Educating and informing students and families informally about mental health, addiction, co-occurring disorders, stigma, fear, and ways to help oneself or friends/family members who may be struggling with any of these issues; and how CN Guidance can assist with this
  • Providing brief intervention screening and therapy (a few examples include administering surveys that help identify depression, such as PHQ-9; performing SBIRT [Screening, Brief Intervention, and Referral to Treatment]. Must be SBIRT trained.
  • Providing structured, community-wide presentations tailored toward various aged audiences;
  • Reviews, develop and interpret quality assurance standards: o Maintains current regulatory guidelines for program and monitor program activities against these guidelines; o Prepares statistical and narrative reports as required; o Reviews clients' records, applying quality assurance and/or utilization review criteria to determine need for admission and continued stay in program.
  • Participate in school meetings related to coordination of mental health needs for students and the school district
  • Complete QI Training during onboarding and participate in QI refresher training as needed.
  • Ability to cultivate a culture of inclusion for all employees that respects their individual strengths, views and experiences. A Culture that makes better decisions, drives innovation, and delivers better business results.

  • Additional duties as assigned.

EDUCATION AND EXPERIENCE:

  • Master's Degree in Social Work, Mental Health Counseling or Marriage and Family therapy; and New York State licensure.
  • 2 years' experience of post-graduate clinical work in relevant settings including experience with children and families. Familiarity with the school system, supports available both internally and externally of the school district, and knowledge and awareness of community providers for youth and families.
