Job Summary

The Substance Use Navigator is a Certified Recovery Specialist and member of a multidisciplinary team working to address the disease of addiction within treatment programs. The Substance Use Navigator will use their lived experience to initiate a recovery-oriented conversation with persons admitted to the ED and promote a safe non-judgmental engagement to explore treatment options. Will collaborate with treatment providers to ensure a warm handoff to treatment services they need to obtain and sustain recovery.

Job Duties

  • Will serve as a role model, mentor, advocate, and motivator to individuals seeking recovery and provide peer recovery support to help to prevent relapse and promote long-term recovery.
  • Meets with patients and appropriately engages them to identify interests, strengths, goals, dreams, and aspirations while offering encouragement and empowerment through shared experience to enhance strengths and capabilities for members.
  • Identifies, develops, and maintains positive linkages with treatment providers, community agencies and social service organizations.
  • Build strong relationships with outpatient partners and be experts in the local treatment ecosystem.
  • Communicates all concerns to clinical team and manager as necessary, seeking support and guidance proactively.
  • Participates in multidisciplinary treatment team huddles to maximize the treatment plan.
  • Maintains timely and accurate documentation and tracking of all service provisions and treatment outcomes.
  • Maintains Certified Recovery Specialist certification.
  • Appointment scheduling and clerical duties as needed.
