Job Summary

Responsible for developing, implementing, and evaluating an operational plan in physician practices that supports the clinical quality and patient safety strategies and initiatives. 

 

COMPANY DESCRIPTION

Frye Regional Medical Center, located in the beautiful foothills of North Carolina is a 355-bed acute care hospital, 81-bed Behavioral Health facility and more than 70 primary and specialty care providers.

 

Essential Functions

Develop, implement, monitor, and evaluate plans to achieve quality and patient safety improvement goals

Engages providers in strategies to improve outcomes through collaborative partnerships

Responsible for the implementation of the LifePoint Foundational Five quality improvement processes and other LifePoint quality and patient safety initiatives

Serves as a Patient Safety Champion to infuse a culture of patient safety, including an increased awareness and identification of patient safety concerns

Ensures the development and delivery of nursing education/competencies to all clinical staff

Manages high risk patient population and functions as a coordinator across multiple care settings to assure appropriate utilization of clinical and community resources to drive patient safety, clinical quality, and improved outcomes.

Develop an annual patient care plan to ensure all quality care metrics are addressed for attributed patients; Create and maintain patient tracking and productivity reports to facilitate care coordination and identify patients with care opportunities (i.e. Annual Wellness Visits, clinical gaps in care); Coordinate with patient scheduling, facilitate and coordinate patient outreach and facilitate pre-visit screening to promote patient adherence and to close quality care gaps.

Work with vendors and external partners to obtain beneficiary, utilization, and quality data; Responsible for data collection, dashboards, and management of data delivery to providers and all other stakeholders; Use analytical reasoning to measure the impact on key performance and quality metrics.

Work collaboratively with providers and practice staff to infuse a culture of accountability, steadfast quality focus, teamwork, coordination, transformational workflows, continual improvement, and care innovation to contribute to the quality and patient safety initiatives.

Knowledge of continuous quality improvement principles, practices, methods, and tools; Actively participates or leads Performance Improvement Teams

Skilled in effective education, coaching, team building and facilitation skills

Direct and drive the implementation, training and hardwiring of tactics and evidence-based practices to improve patient/family experiences, engagement and satisfaction.

Technology skills include proficiency with the electronic health record, specifically the clinical record, basic excel and use of payer database systems. Responsible for working side by side with clinical support staff and providers to train and optimize the use of the EHR to document and close quality metrics.

Serves as a Quality Cohort Team Member to provide consultative guidance in the development and implementation of a National Quality Program for Ambulatory Physician Practices.

Perform other duties as assigned.

Minimum Education

Associate’s Degree in Nursing required

Bachelor's Degree in Nursing preferred

 

Required Skills

Requires critical thinking skills, decisive judgment and the ability to work with minimal supervision. Must be able to work in a stressful environment and take appropriate action.

Licenses: Current RN license to practice in NC (NC licensure or multi-state (compact) license). Certifications: CPPS or CPHQ preferred (Certification required within one year)

Minimum Work Experience

Minimum of 2 years in a clinical practice setting; prefer Provider Practice Management or Patient Safety
