Our facility in Wilkes-Barre, Pennsylvania, is seeking a compassionate and dedicated Long Term Care Staff Pharmacist to join our dynamic team. Job Description:
As a Long Term Care Staff Pharmacist, you will play a crucial role in the management of medication therapy and quality care for our residents. Your responsibilities will include but are not limited to:

• Reviewing prescription orders to ensure accuracy and suitability of the medication for the resident.
• Providing pharmacological information to the multi-disciplinary healthcare team and participating in the decision-making process regarding patient health.
• Monitoring patient medication regimens for contraindications, drug-drug interactions, drug-food interactions, allergies, and appropriateness of drug and dose.
• Managing and overseeing the medication dispensing process to ensure compliance with federal, state, and local regulations.
• Advising on the correct administration of drugs, including injections, infusions, and inhalations.
• Implementing health and safety policies in handling and dispensing medications.
• Engaging in medication therapy management and clinical interventions to optimize patient outcomes.
• Maintaining accurate records of all controlled substances, ensuring compliance with regulatory audits.
• Educating patients and caregivers on medication-related matters.
• Leading and training pharmacy technicians and support staff.

Shift:

• Monday - Friday: 11:00am - 7:00pm
• Possible weekends.

Qualifications:

• Pharmacy experience.
• Pennsylvania Pharmacy license.
