Job Description:
As a Long Term Care Staff Pharmacist, you will play a crucial role in the management of medication therapy and quality care for our residents. Your responsibilities will include but are not limited to:
- Reviewing prescription orders to ensure accuracy and suitability of the medication for the resident.
- Providing pharmacological information to the multi-disciplinary healthcare team and participating in the decision-making process regarding patient health.
- Monitoring patient medication regimens for contraindications, drug-drug interactions, drug-food interactions, allergies, and appropriateness of drug and dose.
- Managing and overseeing the medication dispensing process to ensure compliance with federal, state, and local regulations.
- Advising on the correct administration of drugs, including injections, infusions, and inhalations.
- Implementing health and safety policies in handling and dispensing medications.
- Engaging in medication therapy management and clinical interventions to optimize patient outcomes.
- Maintaining accurate records of all controlled substances, ensuring compliance with regulatory audits.
- Educating patients and caregivers on medication-related matters.
- Leading and training pharmacy technicians and support staff.
Shift:
- Monday - Friday: 11:00am - 7:00pm
- Possible weekends.
Qualifications:
- Pharmacy experience.
- Pennsylvania Pharmacy license.
