Client location and schedule hours:
Client is located in Franklin- Schedule Tuesdays and Saturdays 10 am to 3 pm
Job Responsibilities:
Social History: Male client He has always been very athletic. Most of his life he coached High School baseball and football. He loves to exercise and moves around a lot to keep from getting stiff muscles but must rest between activities. client uses a walker all through the house. He's a quiet and reserved man and seems to have good receptive and expressive communication skills although they appear slightly delayed.

Diagnoses: Cervical Myelopathy (related to a spinal cord injury), Orthostatic Hypotension (BP issues), Dementia, Pacemaker (2021)

Goals & Objectives: Assist client with his shower and toileting as needed. Walk with him on all transfers and observe for weakness and have him rest or sit down. Engage with client and remind/offer him hydration often. Spot him during exercise. Light housekeeping, change bed linens weekly. Do laundry as needed.

Mobility/Transfers: Clientuses a walker for all steps he takes inside the house.

Showering/Toileting/Dressing: Client takes a shower and shaves every other day. he has a hand held shower head and support bars. Client needs help getting dressed and may need to rest along the way. He can help pick out his clothes.

Toileting: Clientis a private guy, CG should do standby assist and remind him to move slowly.

Eating:Client can feed himself but needs to have meals prepared and served and he needs to be encouraged to eat.

Qualifications:
Minimum of 1 year experience as an in-home Caregiver, CNA or related experiences with a loved one of family member
Ability to effectively communicate through written and verbal language
Valid Driver's License and adequate auto insurance
Consent to criminal, elder abuse, motor vehicle and sex offender background checks
Consent to drug and alcohol testing
TB test
