Summary:

The Sr. Quality Assurance Performance Improvement Nurse is responsible for collecting, processing, and coordinating information and activities pertaining to quality and performance improvement for all clinical departments reporting to the Home Care Director. Responsible for achieving, reporting, and maintaining all regulatory and accreditation requirements in Home Care and other areas of responsibility as assigned. The Sr. Quality Assurance Performance Improvement Nurse will be responsible for monitoring, auditing, and educating on compliance standards as well as developing and maintaining action plans for any needed compliance correction. The Sr. Quality Assurance Performance Improvement Nurse is responsible for the surveillance, identification, prevention, control, and investigation of infectious and communicable diseases under the Home Health Care umbrella.

Responsibilities:

  1. Coordinates quality and performance improvement processes for the clinical departments in coordination with leadership, using evidence-based improvement science practices, including compliance with applicable regulatory and accreditation bodies.
  2. Collaborates with Analytics to analyze and evaluate data and clinical information and compares to data from external sources. Uses data to identify, trend, prioritize and implement improvement projects and ensure all assigned departments meet key performance metrics.
  3. Assists in the development, evaluation and implementation of the Quality Assurance and Assessment Program, QAPI Program and projects, Department of Health Plans of Corrections and restorative nursing programs.
  4. Develops and leads interdisciplinary teams to implement Performance Improvement Projects (PIPs).
  5. Assists in providing in-service training and continuing educational programs to nursing staff in accordance with federal, state and local regulations and Home Health policies and procedures.
  6. Effectively interacts with staff, residents, their significant others and the ancillary health care team members in accordance with Akron Children’s Home Care.
  7. Serves as the electronic medical records point person for facilitating training of nursing staff, providing training when new computer procedures are developed and resolving issues that arise from the Medical Records software use at Akron Children’s Home Care.
  8. Provides annual continuing educational programs for staff as required by the state, federal and local agencies to maintain compliance with licensure.
  9. Participates and assists in ensuring compliance with surveyor pathways used by State and Federal surveyors to prevent various being cited.
  10. Participates in Care Plan Meetings as needed depending on the specific issue of concern.
  11. Leads interdisciplinary quality and performance improvement teams, reporting out to various department, organization, and contracted entities.
  12. Monitors clinical, operational, and organizational performance to ensure safety and promote quality outcomes for the patient, family, and staff.
  13. Mentors and collaborates with department staff to implement evidence-based performance improvement practices.
  14. Acts as a change agent to promote a culture of continuous improvement, putting patients/families at the center of all activities.
  15. Understands, adheres to and educates the department in regulatory standards, as they apply to performance improvement, regulatory and accreditation requirements, and departmental key performance metrics.
  16. Assists in the development, evaluation and implementation of the Quality Assurance and Assessment Program, QAPI Program and projects, Department of Health Plans of Corrections, and restorative nursing programs.
  17. Provides in-service training, governmental required annual continuing educational programs, and continuing educational programs to staff in accordance with federal, state, and local regulations as well as hospital and departmental policies and standards of care.
  18. Other duties, as required.

Technical Expertise

  1. Experience with project management for interdisciplinary teams is required.
  2. Experience working with all levels within an organization, demonstrated initiative, and good professional judgement is required.
  3. Ability to work independently and with minimal direct supervision and as a team member.
  4. Clinically competent in post-acute care and skilled nursing care, professional, and a licensed registered nurse who uses the nursing process to reduce risk and develop interventions to prevent resident incidents from occurring.
  5. Critical and analytical reasoning, astute clinical judgement, strong communication (verbal and written) interpersonal, organizational, and leaderships skills required.
  6. Exceptional capacity to deal successfully with a variety of individuals and ability to work with and provide care and services to difficult people.
  7. Demonstrates professional, appropriate, effective, and tactful communication skills, including written, verbal and non-verbal.
  8. Proficiency in MS Office [Outlook, Excel, Word, PowerPoint] or similar software is required. Epic software or similar EMR software is preferred.

Education and Experience

  1. Master of Science in Nursing degree with a current unrestricted license as a Registered Nurse in Ohio is required.
  2. A minimum of 5 years of relevant clinical experience in nursing required. Pediatric ambulatory, quality/performance improvement science, population health, care management, long term care, post-acute care, skilled nursing care and/or health plan experience is preferred.
  3. Basic Life Support (BLS) certification from the American Heart Association is required.
  4. Certification by a nationally recognized care management or performance improvement organization preferred.
  5. Completion of a standardized quality/performance improvement and/or evidence-based practice implementation course preferred (i.e., IQI, IHI, EBP Immersion, etc.) required or must be obtained within one year of hire.
