Inpatient Pharmacist Newark, DE

We are seeking a licensed Pharmacist in Newark, DE for a full-time 13 week contract, in an inpatient hospital setting.

Schedule: Full-time, 1st shift, Monday Friday (7:30am 4:30pm); every 3rd weekend rotating

Responsibilities:

• Participate in Direct Patient Care.
• Manage Medication Therapies.
• Evaluate Drug Efficacy.
• Order and Evaluate Laboratory Tests.
• Medication History and Counseling.
• Communicate directly with healthcare providers regarding medication management and therapy adjustments.
• Provide drug information to other healthcare professionals and responding to provider-initiated.
• Responsible for satellite pharmacies, which may include tasks such as sterile product processing in the IV Admixture Service and OR pharmacy.

Requirements:

• B.S. in Pharmacy required; Pharm.D degree preferred.
• ASHP-accredited PGY-1 Pharmacy Practice Residency preferred; equivalent experience will be considered (3 years of acute care hospital experience).
• Board certification preferred.
• Current licensure in state of Delaware or obtainable before initiation of employment.
