Join our exceptional Optum Palliative Care team.  As a Nurse Practitioner on the Hospice and Palliative Care Team, you will provide primary care and palliative care to patients in the hospital setting and occasionally in their homes or in a facility. This excellent opportunity affords a flexible, autonomous role bringing enormous satisfaction in the care and comfort of our aging population. Additionally, you will build meaningful relationships with the residents and their families. 

 

This Nurse Practitioner position is with our Optum Palliative Care and Hospice Program (previously known as Evercare Hospice and Palliative Care). The Nurse Practitioner will collaborate with the Executive Director, Medical Director and Clinical Services Manager to ensure that all patients are seen in a timely manner. 

 

Please note: This is a full time position that will require travel throughout our service area (Maricopa County). Average case load size will vary based on census. Patient visits will be primarily in the home or facility setting but occasional hospital visits may also be part of the role.

   

Key responsibilities of the Palliative Care NP include:


  • Providing palliative care consults for patients in a variety of settings including in home, facilities and/or the hospital setting.

  • Visiting patients in their homes/facilities on an as needed basis to conduct follow up assessments and provide palliative care services

  • Ongoing collaboration with the PCP and Medical Director regarding palliative care for the patients in our census

  • Collaborating with the nursing staff and the patients' families to ensure quality of care

  • Educating patients and their families about palliative care services provided

  • Participating in an on call schedule (as needed)

Requirements

Qualifications:


  • Current, unrestricted NP license in the State where this position is located

  • Nurse Practitioner national certification as ANP, FNP and/or GNP

  • A clinical background in adult, family or geriatric specialties

  • DEA or ability to obtain

  • Prescriptive authority or ability to obtain

  • Minimum 1 year NP experience

  • Ability to work autonomously

  • Reliable transportation and insurance

  • Intermediate level of proficiency in PC based word processing including Microsoft Word and Outlook 

Additional Assets Preferred:


  • Long term care and/or nursing home based experience

  • Palliative care experience 

  • Hospice experience

  • Home care experience

  • Hospital based experience

  • Oncology experience

  • Criticial care experience
