Position Description:

Candidates within this position will be responsible for reaching and engaging assigned existing non-compliant health plan members telephonically, in person and/or through the member's PCP. Some in-person contacts with each assigned member are required.

As well, candidates will be connecting members with physicians, provider offices, community resources and/or clinical services as appropriate and serving as a member advocate in overcoming identified barriers to care which will include connecting members with community resources, assisting members with transportation, or locating translators. Candidates will be responsible for documenting all activities in the appropriate clinical management system.

Candidates need to be able to adapt to an evolving position.  Candidates will have a goal of seeing 6 members per day within their homes three times a week which will equate to 18 home visits a week.  In addition, 30% of the time candidates will be calling members outbound (goal is 160 calls a week).

Requirements

Required Qualifications:
  • High School Diploma/GED.
  • Ability to travel 70% of the time within the Phoenix area.
  • Valid Driver's License, reliable transportation, and applicable insurance required.
  • 5+ years of behavioral health/medical or healthcare/social work experience required.
  • 1+ year of experience working with MS Excel, Word and Outlook
  • 1+ year of experience working with customers in person or over the phone.
  • Bilingual in English and Spanish required.
Preferred Qualifications:
  • Bachelor's Degree preferred.
  • Case Management background preferred.
