We are seeking a knowledgeable, compassionate RN to provide direct care to hospice patients in an inpatient unit within the hospital. As the Hospital Liaison, you would network with hospital case managers and staff to build relationships in order to establish ourselves as a hospice service provider for patients. The Hospice Hospital Liaison will coordinate admissions, referrals and meetings with families to discuss hospice services. This position offers potential for growth, a friendly staff, and an opportunity to interact with patients and their families on a more personal level.


 


Please note: This is a full time position. The Hospice Liaison will primarily work out of local hospitals in the Alexandria and Mt. Vernon areas. The Liaison may also be requested to visit hospice patients in their homes or a facility setting in order conduct admissions and/or follow up visits.  


 


Responsibilities for the Hospital Nurse Liaison include:



  • Visiting local hospitals in order to build and foster relationships with hospital case managers and staff

  • Coordinating, assessing, and implementing patients' care prior to discharge

  • Providing accurate and timely documentation of patient services to reflect plans of care

  • Discharge planning, coordinating admissions, referrals and meeting with families to discuss hospice services

  • Attending weekly interdisciplinary meetings

  • Performing physical activities that require considerable use of your arms and legs and moving your whole body, such as climbing, lifting, balancing, walking, stooping, and handling of biohazard materials

  • Participating in a rotating on call schedule which may include (nights, evenings, weekends and holidays)

Requirements

Required Qualifications:



  • RN licensure (current, unrestricted) in the state location of position

  • 3 years of clinical experience

  • Reliable transportation including a valid driver's license and auto insurance

 


Preferred Qualifications:



  • Excellent communication skills

  • Marketing, sales and/or business experience

  • Hospice experience

  • Hospital experience

  • Experience in long-term care

  • Computer proficiency
