Team members will be responsible for researching and providing community resources to Care Advocates who will share the information with their members. Team members will complete searches using clear and appropriate verbal and written communication, complete on average 28 searches per week, and perform other duties as communicated by supervisor.


Primary Responsibilities:

  • Provide referral information to WorkLife Specialists, EAP counselors, and Care Advocates within specified time period or 48 hours of intake if not defined
  • Research and resources specifically to meet members needs
  • Screen and verify resource through telephone interview on each case
  • Format resource information into referral template conforming to UBH protocols 100% of the time
  • Maintain a weekly caseload based upon current defined metrics and standards.
  • Maintain computer database of resources
  • Update database each time resource is contacted
  • Develop and maintain national database of resources utilizing Internet, web sites, software programs, resource directories and publications on a daily basis
  • Accurately follow complaint procedures and document complaints against providers and resources within one hour of receipt of complaint
  • Represent Optum at meetings, conferences as required.
  • Participate in monthly team meetings
  • Interface with provider organizations as needed
  • Provide information through consultation and educational materials to providers and community resources as needed
  • Assist service delivery team and other Optum departments as needed
  • Maintain and improve knowledge of research skills
  • Participate in at least 4 hours per quarter of off-site training, observation, or workshops.
  • Participate in Optum in-service and conference trainings

Requirements

 Required Qualifications:

  • Bachelor's Degree.
  • Proficiency using Internet Search protocols and knowledge of how to access community resources.
  • Ability to create, copy, edit, send, and save documents utilizing Microsoft Word, Excel & Outlook.

Preferred Qualifications:

  • Bachelor's Degree (or higher level of educational attainment) in Geriatrics, Psychology, Sociology, Human Services, or a related field.
  • Experience within the Adult Elder Community Resource field, either working directly with Senior citizens, or services that support Senior citizens.
